You have probably been told two different things about sugar, and both of them sounded reasonable at the time.

The first is that sugar is sugar. A gram from an apple and a gram from a candy bar are chemically identical, so stop pretending the apple is virtuous. The second is that fruit sugar is natural sugar, which makes it a different thing entirely, so eat all the fruit you want and worry only about the processed stuff.

Here is the frustrating part. The first claim is chemically true and practically useless. The second is practically useful and chemically wrong. Once you can hold both of those at once, the whole label aisle gets easier to read.


The Molecule Is the Same. That Part Is Genuinely Settled.

Food labels list an enormous number of sweetener names, but your body is not meeting dozens of different substances. Nearly all of them break down into a very short list of sugars.

  • Glucose, which also appears on labels as dextrose. This is the sugar your blood carries and a building block of starch.
  • Fructose, sometimes called levulose or fruit sugar. It is handled largely by the liver and it shows up in fruit, honey, table sugar and high fructose corn syrup alike.
  • Sucrose, which is plain table sugar: one glucose molecule joined to one fructose molecule.
  • Maltose and lactose, the sugars of malted grains and of milk.

The fructose in a peach and the fructose in a soda are the same molecule. Your small intestine has no way of checking where it came from, and no enzyme that treats an organic version differently. This is why the sentence “natural sugar is fine, man made sugar is the problem” does not survive contact with the chemistry.

It also means the honey, maple syrup, agave nectar, coconut sugar and molasses sitting in the health food aisle are not exempt. Under U.S. labeling rules, when any of those are used to sweeten a food, their sugar is counted in that product’s Added Sugars line, exactly like cane sugar would be. The words natural, organic and raw describe where an ingredient came from and how it was processed. They do not describe what happens after you swallow it.

The principle: the difference between fruit and candy is not the sugar. It is the package the sugar arrives in, and how fast that package lets it go.

What Actually Changes Is the Package

An intact piece of fruit is a delivery system, and a fairly slow one. The sugar is held inside cell walls, diluted in a large volume of water, and bound up with fiber, potassium, vitamin C and a range of plant compounds. You have to break all of that apart with your teeth, which takes time and creates a real sense of having eaten something.

A medium apple carries somewhere close to 20 grams of sugar, which surprises people. It also carries about 4 grams of fiber and a great deal of water, and it takes several minutes of steady chewing to finish. Very few people eat four apples in a row. Raspberries make the point even harder: gram for gram they carry more fiber than sugar, which is close to the opposite of how a sweetened food is built.

Now compare a 12 ounce sweetened drink holding 42 grams of added sugar. That is roughly 10 and a half teaspoons, and about 84% of the Daily Value the FDA uses on the label. It arrives in about two minutes, requires no chewing, brings no fiber, and does remarkably little to make you feel full. If the bottle happens to hold two servings and you drink all of it, every one of those numbers doubles.

Same family of molecules. Completely different experience for the body absorbing them, and a completely different likelihood that you go back for more.

This is also why the honest version of the advice is not “fruit is free.” Fruit still contains carbohydrate, and if you are managing blood glucose, especially with insulin or an insulin releasing medication, portion size and your own measured response still matter. The point is not that fruit is weightless. The point is that the packaging does real work.

If this feels familiar, it is the same idea applied one level up. Two meals can hand your body the same protein, carbohydrates and fat and still arrive in completely different condition. Sugar is simply the sharpest case of it, because the raw material is identical down to the molecule.

Juice Is the Test Case

If the package is what matters, then juice is the clean experiment, because juice is fruit with the package removed. The water and sugar stay. Most of the fiber and all of the structure do not. An orange eaten whole and a glass of orange juice are not the same transaction, even when the juice is unsweetened and the carton says 100% fruit.

Public health bodies treat it accordingly. The World Health Organization uses the term free sugars, and it deliberately counts the sugars in fruit juice alongside added sugars and the sugars in honey and syrups, while leaving the sugars inside whole fruit out of the total. That single definition tells you where the line actually falls, and it is not between natural and artificial. It is between sugar that is still inside a food and sugar that has been let out of one.

The same logic applies to fruit juice concentrate on an ingredient list. When a concentrate is used to sweeten a granola bar or a yogurt, it is functioning as a sweetener, and it counts as added sugar. A label reading sweetened only with fruit juice is a marketing sentence, not a nutritional exemption.

Three Numbers Worth Knowing

These come from different organizations for different purposes, so they are not interchangeable personal prescriptions. They are useful as reference points.

One useful conversion: a teaspoon of granulated sugar is about 4 grams. It turns an abstract number on a panel into something you can picture.

Myth vs Reality

“Fruit sugar is a different, gentler kind of sugar.”

The molecules are the same ones found in table sugar. What differs is the whole food around them, which is a real advantage but a different claim than the one usually made.

“Honey and agave are natural, so they do not count.”

When used to sweeten a food they count as added sugar on the label, the same as cane sugar. They may differ in flavor and trace compounds. None of them is a free pass.

“If I am watching my blood sugar, fruit is off limits.”

Fruit contains carbohydrate and portion matters, particularly for anyone using insulin. But glycemic index by itself is not a reason to eliminate every higher ranked fruit, and blanket exclusion lists are not individualized care. This is a conversation for you and your clinician, not a rule from an article.

The 30 Second Label Check

You do not need to memorize the sweetener glossary. The current Nutrition Facts label already does the aggregating for you.

1. Start with the serving size. Decide how much of this you will realistically eat or drink, then scale every number to that.
2. Find “Includes X g Added Sugars.” This is the single best line on the panel. It gathers every sweetener in the product into one number, no matter how many different names they used.
3. Read the % Daily Value beside it. Roughly, 5% or less is low and 20% or more is high.
4. Glance at the first few ingredients. Ingredients are listed by weight. A sweetener near the front, or several sweeteners scattered through the list, tells you sweetness is a main feature of the formula.
5. Compare like with like. Match serving sizes between two products before deciding which one wins.
6. Start with drinks. Sweetened beverages deliver the largest dose in the least time with the least fullness, which makes them the highest yield thing to change first.

Two things you will meet on labels that are not added sugar, and are worth filing separately. Maltodextrin is a digestible starch rather than a sugar, so it never appears in the Added Sugars line at all, and it can still raise blood glucose quickly. This is why Total Carbohydrate is the more useful number if you are managing glucose. And artificial sweeteners are not sugar and are not counted here either. Whether they are a good trade is a real question, but it is a separate one with its own evidence, not a sugar question.

Your one action this week

For three ordinary days, do not change anything. Just write down the Added Sugars grams for everything you drink. Not what you eat, only what you drink.

Most people are surprised, and drinks are the easiest category to fix without feeling like you gave anything up. When you are ready to swap, start at the Dirty List.
